UnitingCare Queensland seeks a Supported Living Facilitator to provide in-home care, helping clients live independently and improving daily living.
You will travel between homes, supporting activities and following Care Plans to enhance quality of life.
A current CPR/First Aid certificate, NDIS Worker Screening, and a valid driver's license with vehicle access are essential.
Work mainly Monday–Friday, 9am–5pm in Caloundra.
