Essential Criteria A recognised qualification in Social Work, Allied Health, Community Services or a related field under the Support at Home Manual; or current AHPRA registration as a Division 1 or 2 nurse Ability to travel for community visits and work onsite Knowledge of Home Care Packages/Support at Home, My Aged Care and the Aged Care Quality Standards Commitment to helping older people live independently at home Excellent communication, planning, organisation and time management Confidence with Microsoft Office and participant management systems Ability to build strong relationships with teams and service providers Current First Aid and CPR certificates Current Victorian driver's licence Desirable Criteria Experience in Home Care Packages/Support at Home or government-funded health or social services Experience with Carelink Plus or similar client management software Experience supporting diverse older people, including those with complex needs Experience in assessment, care planning and community service coordination Robust negotiation and collaboration skills with community providers Why work for us?
