What you'll be doing




Manage a portfolio of clients and build
genuine, trusted relationships
with clients and their families.
Complete assessments and develop personalised care plans based on each client's
needs, goals and preferences
.
Use your
Support at Home and HCP knowledge
to plan and coordinate services effectively within available funding.
Coordinate caregivers, allied health professionals and other providers to deliver consistent, high-quality care.
Monitor client wellbeing and proactively review care as needs and circumstances change.
Manage client budgets and funding with a focus on sustainable, appropriate service delivery.
Maintain accurate care plans, documentation and funding records.
What we're looking for
We're looking for someone who is
caseload-ready and can make an immediate contribution
.
You'll have:
Demonstrated experience as a
Care Manager, Case Manager or similar aged care professional
.
Strong, practical experience with the
Support at Home program
, including funding, budgets and service coordination.
Experience managing a
client caseload
and coordinating multiple services and stakeholders.
A strong understanding of
person-centred care
and the ability to turn individual needs and goals into practical care plans.




Excellent communication and relationship-building skills — you're a genuine
people person
who can build trust with clients, families and care teams.
The confidence to have sometimes difficult conversations while keeping the client's best interests at the centre.
Strong organisation and attention to detail across care plans, budgets, documentation and competing priorities.
A practical, solutions-focused approach and the ability to work independently while contributing positively to a team.
