Wunan Health in Western Australia seeks an Aboriginal Health Practitioner (AHP) to join its multidisciplinary team, delivering culturally protected, person-centred primary care within scope of practice and in line with best available evidence.
You will provide triage, immunisations, wound care, ECGs, health assessments, chronic disease monitoring and care coordination with GPs, nurses and allied health to support coordinated, holistic care for individuals and families in the community.
