St John of God Midland Private Hospital is seeking a Discharge Coordinator on a fixed-term full time basis from 30 November 2026 to 1 August 2027. The role focuses on delivering patient-centred discharge planning and improving outcomes in collaboration with nursing leadership.

You will maintain NMBA competency, lead education for the multidisciplinary team, and advance clinical practice while supporting our Mission and Values.
