Due to continued growth, our client is seeking experienced House Team Leaders to oversee Supported Independent Living (SIL) homes across Perth Metro and Port Hedland. This hands on role combines clinical expertise, team leadership and operational management to deliver exceptional participant outcomes.

The Role

Reporting to the Service Delivery Lead, you will:

- Lead and support a team of Support Workers
- Oversee daily house operations and service delivery
- Coordinate participant care and support services
- Maintain care plans, documentation and compliance requirements
- Manage medication processes and clinical governance activities
- Build strong relationships with participants, families and stakeholders
- Drive quality,



safety and continuous improvement

About You

- Current AHPRA registration as an Enrolled Nurse (EN)
- Minimum 3 years' experience in disability, healthcare or community services
- Demonstrated leadership or team coordination experience
- Solid understanding of NDIS standards and person-centered care
- Experience with care planning, documentation and stakeholder engagement
- Current Driver's Licence, First Aid, CPR and NDIS Worker Screening Clearance

Highly Regarded

- SIL or residential disability experience
- Previous experience as a House Team Leader, Team Leader or Senior Support Worker
- Certificate IV or Diploma in Disability, Community Services or Leadership & Management
- Knowledge of Positive Behaviour Support, mental health or complex care
