Mission Australia’s Marapikurrinya South Hedland service is seeking a Case Manager to deliver person-centred support to older people with complex needs, helping them access aged care and community supports, navigate systems, and build independence through outreach and face-to-face engagement.

You will manage a caseload of 20–30 clients, work in a remote Pilbara setting, collaborate with health, housing and community services, and uphold privacy, trauma-informed practice, and robust relationship
