Juniper in Perth North Metro is seeking a culturally responsive Care Partner to join our Home and Community Care team. You will coordinate SAH and CHSP programs, build solid relationships with customers and families, and ensure care is delivered with respect.

You’ll lead Community Support Workers, monitor client health, and drive trauma-aware, healing-informed care while complying with the Aged Care Act 2024 and Juniper policies. This role demands compassion, teamwork and a hands-on approach.
