NSW Health is seeking a Community Nurse to deliver person-centred nursing care in Boorowa and surrounding communities. You will work with patients, families and multidisciplinary teams to support chronic disease management, recovery and independence, helping clients remain safely at home.

Accountabilities include holistic nursing assessments, care planning and coordination with GPs, carers and other professionals.
