Calvary Care is seeking a dedicated Care Manager (Full Time) to work in the community, with a base in Mulgrave. You will collaborate with clients, families and stakeholders to deliver person-centred care and manage care plans, budgets and service delivery.

This full time role requires prior care management experience in Home Care, EN or RN, and a Diploma/Degree in Health or equivalent. A current Australian driver’s licence and reliable vehicle are essential.
