Everyday Independence in Albury is seeking a Youth Lead Practitioner to support adolescents and young adults aged 9-25 with disability. You will coordinate an interdisciplinary team to plan NDIS-funded therapy packages and act as the main contact for participants and families.
You'll apply assessment and intervention skills to help participants reach educational, personal, social and work-life goals, across settings from home and school to community centres.
