CatholicCare Sydney is seeking a Clinical Care Partner to support the clinical care needs of Home Care Package clients in Sydney. You will work with clients, families and care teams to develop and implement personalised care plans and coordinate services with carers and external providers.

Ideal candidates have 3+ years’ aged care experience, Nursing or allied health background, and robust leadership and communication skills.
