Sydney Children's Hospitals Network is seeking a Health Clinician Case Manager to support children with complex physical and cognitive disabilities in the Westmead rehab program. Working with a treating team, you will develop and implement rehabilitation plans and lead outreach to families, schools and community clinicians.

You will participate in teaching, research and quality assurance activities within rehab and SCHN, with adaptable work options and comprehensive salary packaging.
