About Proactive Care

Proactive Care is the GP-led integrated care service of Proactive Health. Proactive Health is a national network of more than 40 allied health businesses operating across 450+ locations in Australia.

We deliver structured chronic disease management for veterans and for patients with complex, multi-system needs. The work runs through integrated Chronic Care Teams across Medicare and DVA billing streams. Every patient has a team around them, and the GP leads it.

The role

Much of the GP telehealth work on offer today is high-volume scripts and certificates for patients you’ll never see again. This role is built around continuity.

You’ll run structured, GP-led case conferences for patients with chronic and complex conditions, at around 22 reviews over an 8 hour session. Care coordinators organise the team and handle the admin, so your time goes on clinical judgement and care planning.

Your week combines telehealth case conferencing from your home office with clinic days at partner clinic and community sites. Days are flexible and ongoing, and we agree them with you by location.

What you'll do

- Lead GP-led multidisciplinary case conferences from home, typically around 22 patient reviews across an 8 hour session
- Review each patient's progress with the allied health team and set the ongoing care plan
- Integrate pathology, imaging and allied health input into coordinated care plans
- Provide comprehensive consultations for acute, chronic and complex conditions, plus preventive health assessments, on clinic days
- Deliver chronic condition management planning and reviews,



including GPCCMPs
- Manage prescribing, referrals and investigations within the care model
- Provide clinical leadership within a Chronic Care Team of physiotherapy, exercise physiology, dietetics, podiatry, OT and care coordination

Remuneration

- Clinic work: $250 per hour
- Telehealth: $215 per hour
- Chronic Care case conferencing: $250 per hour in clinic, $215 per hour remote
- Or 70% of billings, whichever is higher

The hourly rate is a floor, and it's ongoing. You're paid the greater of the hourly rate or 70% of billings for every session, for as long as you work with us. It doesn't expire after an introductory period.

What you'll need

- FRACGP or FACRRM, with specialist registration in general practice
- Eligibility for a Medicare provider number at the relevant site
- Experience in chronic disease management, preventive health and complex care coordination
- Confidence in prescribing, referrals and investigation management
- Strong multidisciplinary communication
- A suitable home office with reliable internet
- An ABN

Nice to have

- Experience with DVA patients and the CVC Program. It's not essential, because onboarding is provided
- Experience working in or leading multidisciplinary teams
