BDAC is seeking an Intake, Assessment Planning Case Manager to provide culturally secure intake, assessment, planning, and short-term support to Aboriginal and Torres Strait Islander Community members.
You will ensure timely access to services, coordinated care and improved wellbeing outcomes.
Reporting to BDAC programs, the role focuses on homelessness, housing stress, family challenges, and complex circumstances with a strengths-based, community-led approach.
