Support to Thrive Services Pty Ltd in Hervey Bay seeks an experienced Support Coordinator to assist participants with NDIS plans and linking to services. The successful candidate will build capacity, coordinate supports, and work with families and providers to achieve goals.

The role is permanent part-time (20–25 hours) with potential to move to full time. A genuine passion for empowering people with a disability and strong communication are essential.
