Right at Home Sydney St George is seeking a dedicated Family Support Worker to assist families caring for loved ones at home. You will meet with clients, assess needs, and develop support plans aligned with care programs and funding arrangements.

You will collaborate with Care Managers, social workers and allied health providers to deliver practical guidance, emotional support and seamless access to NDIS, My Aged Care and related services. A NSW driver’s license is required.
