Care Connect is seeking an experienced Duty Care Manager to join the national Support at Home team on a full-time maximum-term contract through to October 2027. Based in Bella Vista with hybrid arrangements, you will provide national Duty Care Management support across Victoria, Recent South Wales and Queensland.

You will coordinate care, manage escalations, and work with care teams to ensure safe, person-centred services and continuity of care for clients.
