Sunrise Health Service, an Aboriginal community-controlled health organisation, provides culturally protected primary health care across remote Northern Territory. We are seeking a qualified Diabetes Educator to join our Chronic Care team and deliver education and care through regular outreach to Sunrise communities.

You will work with individuals and families to support diabetes self-management, and contribute to coordinated care planning in a multidisciplinary setting.
