You will work in a rights-based, person-centred framework, coordinating with participants, supporters, and interdisciplinary teams within a Shared Care Team.

The role emphasises care planning, service planning, monitoring and education across streams, with opportunities to collaborate with Clinical Care Partners on complex cases.
