Proactive Health is seeking a GP to lead structured, GP-led case conferences for chronic and complex conditions, combining home-based telehealth with clinic days across partner sites. You’ll run around 22 patient reviews per 8-hour session, guided by care coordinators who handle admin.

Versatile, ongoing days by location, with travel between sites and a focus on clinical judgement and care planning within a multidisciplinary Chronic Care Team.
