HammondCare is seeking a Family Liaison Officer to support families of people living with dementia across hospital to aged care transitions. You will provide pre- and post-placement counselling, assess client needs, and help develop personalised care plans within the HACDSP framework.

You will work from our Parramatta area in a national program funded by the Australian Government, with hybrid work options and opportunities for career development.
