NSW Health is seeking an experienced Clinical Change Manager for Virtual Hospital Care at Central Coast Local Health District. The role leads planning, delivery and evaluation of clinical transformation to support out-of-hospital and virtual care, improving patient flow and outcomes across the district.

You will collaborate with CCHaH and clinical teams to drive innovative care models, monitor benefits and report to senior leaders while ensuring secure, person-centred care delivery.
