Silverchain is seeking a Care Partner to join our Aged Community Care Team in Bunbury and Busselton areas. The role focuses on holistic, person-centred care planning and coordination under the Support at Home Program, with potential light care duties.

The position is full time, Monday to Friday, with options for 7am–3pm or 8am–4pm. You will collaborate with clients, families, and multidisciplinary teams to deliver high-quality home care.
