Crescent Care
Liverpool
Build a SIL program from the ground up, and own its growth.
Crescent Care is a registered NDIS and ACNC not-for-profit based in South Western Sydney. We're registered for Supported Independent Living (SIL) and Specialist Disability Accommodation (SDA). We also deliver high-intensity support, specialist support coordination and allied health, and our portfolio of SDA homes is growing.
We're looking for a senior SIL professional to take full ownership of our referral pipeline, from the first conversation through to a safe, well-matched move-in. This isn't a desk-based intake role. You'll be the person who builds the channels, wins the trust of referrers and turns our homes into thriving, well-matched households.
What you'll own
- Referral growth: build relationships with support coordinators, specialist support coordinators, hospital discharge teams, allied health and behaviour-support practitioners, guardians, advocates and community organisations
- Pipeline and CRM: run a disciplined funnel from enquiry through funding verification, assessment, property visit, compatibility, service agreement and commencement
- Vacancy matching: match participants whose funding, needs, preferences and risks we can support safely and sustainably
- Transitions: coordinate property visits, co-tenant consultation, transition planning and handover with operations and clinical leadership
- Reporting: give leadership weekly visibility of channel activity, qualified opportunities, forecast move-ins and lost-referral reasons
What you'll bring
- 3+ years in NDIS SIL intake, business development or referral development, with a track record of SIL referrals you personally sourced and converted into commencements
- Strong understanding of SIL funding, rosters and ratios, compatibility, tenancy separation and service agreements
- Experience with some of the following: hospital discharge, psychosocial disability, complex health,
behaviours of concern, or guardianship interfaces
- A reputation for ethical, participant-first practice. You know that the right placement matters more than any single placement
- Excellent communication, organisation and CRM discipline
- NDIS Worker Screening Check, WWCC and a current driver's licence (or willingness to obtain them)
- A community language and cultural connections in South Western Sydney are highly valued
Why Crescent Care
- Ready-made foundations: SIL and SDA registration, homes coming online and in-house support coordination and allied health
- A senior role with real influence and a direct line to the executive
- A competitive base salary plus a quality-weighted incentive that rewards successful, sustained placements
- A values-driven not-for-profit focused on choice, dignity and quality
How to apply
Apply with your CV and a one-page cover letter that tells us:
- How many SIL referrals you personally sourced in the last 12–24 months, and how many went on to commence
- A referrer relationship you built from zero
- How you would approach your first 90 days with us
Please don't include any participant-identifying information.
