HelloDr Esk is looking for a Mental Health Nurse to join our friendly, supportive team. You’ll provide person-centred mental health care to the local community, working alongside GPs and allied health colleagues in a clinic where you’re valued and not just rostered.

Position description

Role purpose: To deliver high-quality, evidence-based mental health nursing care to patients under the Country Care Connect program, helping people get timely support close to home.

Key responsibilities:

- Conduct mental health assessments and develop care plans with patients
- Provide clinical support and monitoring for patients with mild to severe mental health conditions
- Deliver education, counselling and self-management strategies
- Coordinate care with GPs, allied health providers and community services




- Support medication monitoring and follow-up in line with GP directions
- Maintain accurate, confidential clinical records and meet reporting requirements
- Participate in team meetings, clinical review and quality improvement activities
- Respond to risk and crisis situations using appropriate escalation pathways

About you:

- Registered Nurse with current AHPRA registration (mental health credentialing or qualifications desirable)
- Experience in mental health, primary care or community health
- Strong communication skills and a caring, non-judgemental approach
- Comfortable working independently and as part of a team
- Current Working with Children Check (Blue Card) and a National Police Check, or willingness to obtain them
- Rural or regional experience is a plus, but not essential
