Catholic Healthcare Limited invites applications for a Care Partner role based at Wyong with client visits across the Central Coast. This permanent full time position (Mon–Fri) offers $48.00 per hour plus superannuation and Km reimbursement ($1.11 per km).

You will deliver relationship-centred care, develop care plans, coordinate with multi-disciplinary teams, educate clients and families, and support discharge planning.
