Uniting NSW.ACT seeks a permanent full time Care Partner in Parramatta to coordinate client-centred home and community care, ensuring plans align with goals and funding. You will build trusted relationships and connect clients with the right services so they stay safely at home and connected to their communities.

You’ll travel between Uniting sites and client locations, maintain records in client management systems, and work with families and teams to deliver safe, responsive services.
