14th October, 2026/n Care Partner (Support at Home) - West Brisbane /n A fantastic opportunity to join an established community aged care provider in West Brisbane, supporting older Australians to remain independent and well within their own homes. /n Ideal for an experienced Care Partner / Care Coordinator, RN or EN looking for a manageable portfolio, strong team support and the opportunity to join an organisation with significant growth on the horizon.

- $85,000-$95,000 depending on experience

- Monday to Friday - permanent full-time

/n Key Responsibilities:/n
/n /n
- Develop and manage individualised care plans with clients, families and health professionals
/n
- Complete assessments and regular client reviews
/n
- Coordinate community-based care and services in line with individual needs
/n
- Monitor client budgets and service utilisation
/n
- Build strong relationships with clients, families and multidisciplinary teams
/n
- Respond to changing client needs and coordinate appropriate supports
/n
- Maintain accurate documentation and high standards of care
/n Requirements:/n
/n /n
- Previous experience in community aged care coordination or case management
/n
- Solid understanding of Support at Home and community aged care
/n
- Strong relationship-building, communication and organisational skills
/n
- Confidence managing your own client portfolio
/n
- Relevant aged care, community services or nursing background
/n
- RN or EN candidates welcome, however a nursing qualification is not essential
/n
- Current Driver’s Licence and relevant industry checks
