We're recruiting an Occupational Therapist to join an established aged care service supporting older Australians across Melbourne's eastern suburbs.
This is a fantastic opportunity for an OT who enjoys working with adults, values independence and wants to make a genuine difference in helping older people remain protected and independent in their own homes.

The position is part-time, working 3 days per week, with a focus on providing Occupational Therapy services to clients accessing the Support at Home program.

You'll manage a community-based aged care caseload, completing assessments, recommending equipment and home modifications, and providing practical interventions to improve clients' independence and quality of life.

Your Caseload You'll work with older adults receiving services through the Support at Home program, helping them maintain their independence, safety and participation in everyday activities.

Your responsibilities may include:

- Functional and Activities of Daily Living assessments
- Home safety assessments
- Falls prevention recommendations
- Assistive technology assessments and prescriptions
- Equipment recommendations
- Minor and major home modification assessments
- Mobility and transfer assessments
- Cognitive and functional assessments
- Education for clients, families and carers
- Liaising with care coordinators and other healthcare professionals




- Clinical documentation and report writing

The focus is on providing practical, individualised recommendations that help clients continue living safely and comfortably in their own homes.

We're Looking For This opportunity would suit an Occupational Therapist who:

- Enjoys working with older adults
- Has an interest in community aged care
- Is confident completing functional assessments
- Has knowledge of assistive technology and home modifications, or is interested in developing these skills
- Can work independently in community settings
- Communicates effectively with clients, families and care teams
- Values person-centred care and supporting independence

Previous experience with Support at Home, Home Care Packages or community aged care would be highly regarded.

Requirements

- Recognised Occupational Therapy qualification
- Current AHPRA registration
- Current driver's licence and access to a reliable vehicle
- Australian working rights
