Lifest HealthCare is a registered NDIS Service Provider delivering disability supports across Southeast Queensland. We are looking for an experienced, resilient, confident and highly organised Team Leader/Area Coordinator to oversee participants and a team of Home Care Workers across Logan and surrounding areas.

This is a hands-on operational role for someone who can take ownership, manage competing priorities, make confident decisions and remain calm and solutions-focused when challenges arise.

About the Role

As a Team Leader/Area Coordinator, you will be responsible for the day-to-day coordination and quality of supports within your area, including:

- Leading, supporting and providing clear direction to a team of Home Care Workers.
- Coordinating rosters and proactively resolving staffing and service-delivery issues.
- Overseeing participant supports, care plans, documentation and compliance requirements.
- Monitoring shift notes, incidents, behaviours, timesheets and service quality.
- Responding to participant and family concerns promptly and professionally.
- Managing difficult conversations and addressing staff performance or conduct concerns when required.
- Building effective relationships with participants, families, Support Coordinators and health professionals.
- Providing hands-on participant support
- Managing multiple priorities, meeting deadlines and following issues through to resolution.

About You You have a passion for wanting to help people who are vulnerable and need support in life. You will be someone who takes responsibility, thinks ahead and gets things done.



You are comfortable working independently while knowing when an issue needs to be escalated.

You will have:

- Minimum three years' experience within the NDIS disability sector, including experience with complex participant needs and behaviours.
- Experience leading frontline disability teams.
- Relevant Community Services, Disability, Social Work or equivalent qualification.
- Strong time-management skills and the ability to prioritise a busy workload.
- Confidence making decisions, solving problems and having difficult conversations.
- The ability to remain calm, professional and solutions-focused under pressure.
- Strong communication, stakeholder-management and documentation skills.
- Experience with rostering and managing staffing challenges.
- Strong technology skills.
- Current NDIS Worker Screening clearance and required compliance documentation.
- Full Australian working rights (please note that this role is not suitable for students or temporary visa holders)
- Current driver's licence, reliable vehicle and insurance, as frequent travel is required.
