About the role

Tender Loving Care Aged Care is seeking an organised and client-focused CHSP Lead to oversee the client journey from initial enquiry and referral through assessment, onboarding, service commencement and ongoing coordination. Working closely with clients, families, My Aged Care, referral partners and internal teams, you will support person-centred service delivery, program performance, compliance and continuous improvement.

Key responsibilities

- Manage CHSP enquiries, referrals, intake, assessments and client onboarding.

- Coordinate Support Plans, service agreements, reviews and reassessments.

- Monitor service delivery, client needs, utilisation and follow-up actions.

- Maintain accurate client records and documentation, including DEX reporting requirements.

- Support contracted CHSP outputs, referral conversion and program performance targets.

- Build strong relationships with clients, families, My Aged Care and community referral partners.





- Represent TLC Aged Care at community events and referral network meetings.

- Monitor compliance, identify risks and escalate service delivery concerns.

- Improve intake processes, reporting accuracy and operational workflows.

About you

- Experience in aged care, CHSP, community services or client coordination.

- Understanding of CHSP requirements, client documentation and DEX reporting, or willingness to learn.

- Robust communication, organisation and stakeholder management skills.

- Ability to manage competing priorities and follow tasks through to completion.

- High attention to detail and commitment to person-centred care.

- Current National Police Check.

- Valid Driver's Licence and NDIS Worker Screening Check, where required.
