Care Manager with a Nursing Qualification (RN)

You won't get bogged down in data entry. Our dedicated Administration and Workforce Rostering teams handle the heavy lifting so you can focus entirely on exceptional client advocacy.

WHAT A TYPICAL WEEK LOOKS LIKE
n
Reporting directly to the Service Manager, you will be the champion for your clients' safety, independence, and happiness at home. Your responsibilities will include:
n
n
Care Planning:
Developing, reviewing, and tailoring comprehensive Care Plans in close consultation with clients and their families.
n
Service Oversight:




Partnering with our rostering team and third-party suppliers to ensure support workers are perfectly matched and scheduled to meet customer goals.
n
Financial Stewardship:
Monitoring individual client budgets to ensure their care plans are accurately funded and sustainable.
n
n
About You
n
You are empathetic, highly organized, and motivated by the idea of making a tangible difference in the lives of older Australians. You thrive in autonomous environments but love being part of a fun, dynamic team culture.
n
Key Selection Criteria:
n
n
Ideally 12+ months of experience in Home Care, Community Aged Services, or Care Management.
n
Experience across Social Services, Aged Care, Disability, or the broader Health Industry.
Registered/Enrolled
Nurses are highly encouraged to apply!
n
Exceptional time management, strong computer/admin literacy, and top-tier communication skills.
n
A skilled, empathetic approach with a knack for exceeding customer expectations.
n

Logistics & Requirements:
n
n
A valid Australian driver's licence and a safe driving record.
n
Your own reliable vehicle (and a willingness to travel to client homes)
