Deliver person-centred nursing care in clients' homes.
n
Conduct nursing assessments, develop nursing care plans, and complete clinical documentation.
n
Provide clinical advice to clients and families, ensuring dignity, choice, and independence.
n
Collaborate with Home Care Managers, Coordinators, and external health professionals.
n
Ensure compliance within Aged Care Quality Standards, legislation, and Lutheran Services policies.
n
n
About You:
n
n
Current
AHPRA registration (Nursing).
n
Strong
clinical assessment
and
care planning
skills.
n
Ideally experienced in
home care & aged care




or community nursing
.
n
Current
Full Australian Driver's Licence
and a
fully comprehensively insured and reliable vehicle
. Must be willing to drive between locations where required to do so.
n
Ability to
support
care partners and support staff with
clinical needs
.
n
Current
National Police Check
(or willing to obtain).
n
