Care Partner (Support at Home) - West Brisbane
A fantastic opportunity to join an established community aged care provider in West Brisbane, supporting older Australians to remain independent and well within their own homes.
Ideal for an experienced Care Partner / Care Coordinator, RN or EN looking for a manageable portfolio, strong team support and the chance to join an organisation with significant growth on the horizon.
Benefits:
- $85,000-$95,000 depending on experience
- Monday to Friday - permanent full-time

Key Responsibilities:
- Develop and manage individualised care plans with clients, families and health professionals
- Complete assessments and regular client reviews
- Coordinate community-based care and services in line with individual needs
- Monitor client budgets and service utilisation
- Build strong relationships with clients, families and multidisciplinary teams
- Respond to changing client needs and coordinate appropriate supports
- Maintain accurate documentation and high standards of care
Requirements:
- Previous experience in community aged care coordination or case management
- Solid understanding of Support at Home and community aged care
- Strong relationship-building, communication and organisational skills
- Confidence managing your own client portfolio
- Relevant aged care, community services or nursing background
- RN or EN candidates welcome, however a nursing qualification is not essential
- Current Driver’s Licence and relevant industry checks
