Coordinate tailored support services that reflect clients’ goals and health conditions
Make decisions that prioritise client safety, wellbeing, and quality of life
Lead and supervise your team of Support Workers, ensuring service quality and continuity
Monitor and manage client budgets to ensure full and appropriate utilisation
Maintain compliance with current legislation, funding guidelines, and internal policies
Collaborate and communicate with the client’s network of supports
Document accurate and timely progress notes on client interactions, changes in condition, incidents, and follow-up actions
Review progress notes submitted by support workers, suppliers, and allied health professionals to ensure coordinated, high-quality care
Admission of current clients with intake assessments and meet and greets
