Western Health is seeking an experienced mental health clinician to join our Community Mental Health Team as a Case Manager. You will provide recovery‑oriented care within a multidisciplinary team, partnering with consumers, families and services to promote safety, wellbeing and community participation.
You will manage caseloads with assessment, care planning and service coordination, build therapeutic relationships, monitor risk and ensure accurate documentation.
