About the Role

You will provide person-centred nursing care to clients in their own homes, helping them maintain their health, independence and quality of life.

Your responsibilities will include:

- Conducting clinical assessments, reviews and care planning.
- Providing wound care and supporting medication management.
- Monitoring clients' health and responding to changes in their condition.
- Supporting clients living with chronic conditions, dementia and complex care needs.
- Collaborating with GPs, allied health professionals and families.




- Providing clinical guidance and support to our Care Partners.
- Maintaining accurate clinical records and reporting incidents appropriately.

You will have the opportunity to use your clinical judgement, develop strong relationships and make a meaningful contribution to the care our clients receive.

About You

You are a Registered Nurse who is passionate about quality aged care and enjoys working independently while remaining connected to a supportive team.

To be successful, you will have:

- Current AHPRA registration as a Registered Nurse.
- Experience in aged care, community nursing or a similar clinical setting.
- Strong clinical assessment, communication and documentation skills.
- Confidence in wound care and supporting clients with chronic conditions and dementia.
- A current driver's licence and CPR certification.
- Relevant police clearance and NDIS screening requirements.
