Five Positive Friends is seeking a Care Specialist to join our care team and support Members with complex clinical needs in home settings. You will work with Care Team Leaders and Coordinators to ensure high-quality assessment, planning and delivery of person-centred care.

You will build trusted relationships with Members and families, providing clinical guidance and coordinating services across multidisciplinary teams using digital platforms.
