Gladstone Community Linking Agency (GCLA) is hiring a Senior NDIS Care Coordinator to lead the delivery of supports for participants with complex needs in Gladstone and Rockhampton. You will coordinate SILS funding, develop individual plans, and work with clinical teams to ensure protected, person-centred services aligned with NDIS plan outcomes.
You will oversee intake, oversee plans, and guide frontline staff while maintaining participant choice, dignity and independence.
