The Rehab@Home service at South West Hospital and Health Service is seeking a Dietitian to provide evidence-based, patient-centred dietetic care across community and residential settings, contributing to rehabilitation and restorative care goals.

You will conduct dietetic assessments, interventions, and evaluations within protocols, collaborate with clients, families, and health professionals, and contribute to education and service development.
