Silverchain is recruiting a Care Partner in Bunbury to support the Aged Community Care Team across Bunbury and Busselton areas. The role focuses on holistic, person-centred care, coordinating care plans and managing day-to-day care activities within the Support at Home program.
You will assess client needs, develop plans, supervise care staff and liaise with GPs and allied health professionals to ensure timely, high-quality care delivery.
