Melbourne Health is seeking a Progression of Care Navigator to support timely patient care and discharge planning at the Royal Park Campus, Rehabilitation Ward. This part-time role coordinates routine and complex discharge plans with patients, families and the clinical team to improve ward access and patient flow.
You will be registered as a Division 1 RN or Allied Health Professional with AHPRA and have at least five years’ sub-acute experience and solid communication with diverse stakeholders.
