Wide Bay Hospital & Health Service in Bundaberg, Fraser Coast, is seeking a Senior Social Worker to deliver high level clinical services to patients on the Transition Care Program as part of the Integrated Care Rehabilitation Service.

The role focuses on rehabilitation of older people after acute care, helping prevent unnecessary admissions and supporting Allied Health Assistants in therapy programs. Two permanent part time positions are available, 38 hours per fortnight.
