HammondCare delivers Dementia Support Australia (DSA) services nationwide to people living with dementia and their families. This role supports aged care staff and carers, helps implement dementia care plans, and coordinates resources to improve quality of life for clients.
You will work within a team, engage with carers, and travel as needed to support service delivery across intrastate and interstate locations. A relevant qualification in Allied Health or Aged Care is highly desirable.
