Wintringham's Williamstown Hostel is seeking an experienced Care Evaluator to join our team, delivering resident-centered care, care planning and hands-on support in a close-knit community.
Reporting to the Residential Site Manager, you will assess residents, develop and review Support Plans with residents and care teams, monitor health and behaviour changes, and provide clinical guidance to staff to maintain high standards of care and ensure a respectful, dignified life for residents.
