Just for u Care is seeking a Care Partner – Support at Home (Case Manager) in Cheltenham, VIC. You will be the main point of contact for a portfolio of older Australians on Support at Home packages, managing the full client journey from assessment through care planning, budgets and reviews.
You’ll work with a team of care professionals, coordinate services with GPs and allied health, maintain documents to meet quality standards, and enjoy hybrid work with 4 days in office and 1 day from home.
