Western Health is seeking a Care Coordinator to join the MHAOD Connect team in Melbourne's western health network. The role delivers social work assessments, counselling interventions and referrals for post-discharge care, working closely with Emergency Department clinicians.
The Care Coordinator undertakes intake, initial needs assessments and provides social work support and case management for clients with AOD and mental health concerns within a multidisciplinary setting.
