About the role

We are seeking a Case Manager to join the Senior Community Care team to provide efficient and effective case management services to clients of Seniors Community Care Services. This role assists older people to remain living safely and independently in their own home and community by facilitating and coordinating the provision of care arrangements that are suitable for their individuals needs.

This role is based at our Somerville office.

As our ideal candidate, you will possess:

- Minimum of Diploma Qualification in Community Services, Social Work and/or Case Management

- Proven experience in case management, preferably in a community, aged care, or multicultural setting

- Relevant demonstrated experience in Aged Care and Carer Support

- Excellent interpersonal, communication, and stakeholder engagement abilities
- A commitment to delivering culturally safe and inclusive services

- Recent experience as a Case Manager of Aged Care Home Care Packages

- Registered Nurse or Enrolled Nurse qualifications are highly desirable but not essential
