Clinical Pharmacist – Grade 1 or Grade 2

Are you a pharmacist looking to develop your clinical skills within a supportive hospital environment? An exciting permanent opportunity is available for a Clinical Pharmacist to join an established healthcare team in regional Victoria, approximately three hours from Melbourne.

This role offers the prospect to deliver high-quality clinical pharmacy services, contribute to medication safety initiatives and collaborate with multidisciplinary healthcare professionals to achieve the best outcomes for patients.

Hospital pharmacy experience is highly desirable but not essential, with training and professional development opportunities available.

The Opportunity

Working alongside an experienced team of pharmacists and pharmacy support staff, you will provide comprehensive pharmacy services across inpatient, outpatient and dispensary settings.

Your responsibilities will include

- Providing ward-based clinical pharmacy services and medication reviews.

- Reviewing medication orders for accuracy, safety and clinical appropriateness.

- Obtaining medication histories and developing medication management plans.

- Dispensing medications accurately and efficiently.

- Providing medication counselling and drug information to patients, carers and healthcare professionals.

- Participating in multidisciplinary ward rounds and supporting seamless transitions of care.

- Contributing to medication safety, antimicrobial stewardship and Quality Use of Medicines initiatives.

- Participating in clinical audits and continuous quality improvement projects.

- Supporting and mentoring pharmacy students and interns.

About You To be successful in this role, you will have:





- A Bachelor of Pharmacy or equivalent qualification from a recognised tertiary institution.

- Current general registration with AHPRA, with no restrictions to your scope of practice.

- Solid clinical knowledge and excellent attention to detail.

- Well-developed communication and interpersonal skills.

- The ability to work collaboratively within a multidisciplinary team.

- A patient-centred approach and a commitment to safe, high-quality healthcare.

- Initiative, problem-solving skills and a commitment to ongoing professional development.

- Strong computer skills.

- A current National Police Check and the required immunisation evidence in accordance with healthcare requirements.

Hospital pharmacy experience is advantageous, but pharmacists looking to transition into hospital practice are encouraged to apply.
